چکیده

BACKGROUND To investigate the efficacy, safety, and patency following treatment of wall-adherent thrombus in hemodialysis vascular access with a wall-contact device, the Arrow-Trerotola percutaneous thrombolytic device (PTD). METHODS We retrospectively reviewed an existing database of procedures fulfilling the following criteria: thrombosed hemodialysis access, wall-adherent thrombus, and use of PTD for mechanical thrombectomy. Data on immediate success, complications, and patency were collected from medical records, dialysis records, and angiographic reports. RESULTS Ninety-three patients with 108 episodes of vascular access thrombosis were included in the study. Fifty-three of the procedures were performed on native fistulas, and 55 were on synthetic grafts. Anatomical and clinical success was achieved in 97% and 96% of the procedures, respectively. The average procedure time was 52 ± 23 minutes. Complications occurred in three of the procedures (2.7%), but none of these complications were device-related. The primary patency rates in the native fistula group were 57% and 42% at three and six months, respectively. The primary patency rates in the synthetic graft group were 40% at three months, and 27% at six months. The secondary patency rates at six months were 91% in the native fistula group, and 93% in the synthetic graft group. CONCLUSIONS Our results show that a wall-contact mechanical device, PTD, is effective and safe for endovascular removal of wall-adherent thrombi in hemodialysis vascular access in both native fistulas and synthetic grafts. Paradoxical embolism after declotting of hemodialysis fistulae/grafts in patients with patent foramen ovale.

BACKGROUND AND OBJECTIVES The safety of percutaneous endovascular declotting procedures for thrombosed hemodialysis fistulae/grafts is well described in the general population; however, its safety in the presence of a patent foramen ovale (PFO) is not known. The objective of this study is to assess the incidence of symptomatic paradoxical embolic events associated with declotting procedure of t...

Invasive radiologic management of hemodialysis fistulas: measuring flow with an endovascular catheter.

OBJECTIVES To analyze the values of flow obtained with an endovascular catheter, and to determine whether they are more reliable than angiographic and clinical findings for planning and for determining the outcome of invasive radiologic treatment of hemodialysis fistulas, as well as to determine the safety of this technique during interventional radiology procedures. MATERIAL AND METHODS We u...

Conservative management of superior vena cava injury due to hemodialysis catheter placement: A case report and literature review

We reported the case of a 78-year old woman with the inadvertent superior vena cava injury resulting from hemodialysis catheter placement. The catheter was placed through the left internal jugular vein. Hemodynamics were stable. The initial chest film illustrated an extraluminal catheter and mediastinal widening. The patient did not have hemothorax. Computed tomography venography also confirmed...

Recurrent white thrombi formation in hemodialysis tubing: a case report

BACKGROUND While the appearance of red clots in the dialyzer is a common phenomenon in every hemodialysis unit, the occurrence of white thrombi in the tubing is relatively rare. CASE PRESENTATION We describe an adolescent male with recurrent white thrombi formation in the hemodialysis tubing. This patient had chronic renal failure from focal segmental glomerulosclerosis, but was no longer nep...
